Pre-Bariatric Surgery Psychological Evaluation: $250 plus $50 letter/report fee.

Cancellation Policy

Insurance based clients please refer to Rula cancellation policy.

Private pay clients please cancel or reschedule at least 48 hours in advance. A fee of $50.00 will be charged for late cancellations or missed appointments; subsequent missed appointments may be charged at the full reimbursable service rate at Kara’s discretion.
